The Osler Institute   
Faculty Agreement on Honorarium, Expenses, Royalties and Content   

The Osler Institute will pay travel, food and lodging expenses and an honorarium as follows:   
I. Faculty Honoraria:   
1.​ A completed W9 form is required in order for The Osler Institute to pay an honorarium.   
2.​ For lectures, mock orals, question and answer sessions and case reviews, the honorarium is $150 per hour of lecture payable in quarter 
hour increments. In the event that a lecture is substantially shorter than the scheduled time, honoraria will be based on actual time spent 
performing the lecture.  An additional honorarium of $150 per hour will be paid for lecture notes for the course syllabus. For Peer Review 
of faculty presentations and/or case studies the honorarium is $30 per hour of  lecture or half-hour of mock oral cases studies; the point of 
the review being to assure said materials are free from commercial bias and conform to ACCME content validation statement.  
3. For any written materials the faculty provide, the faculty certifies that the material is theirs to sell copyright interest in and indemnifies 
the Osler Institute against any claim to the contrary. In exchange for consideration in honorarium and/or other compensation received and 
acknowledged as sufficient, faculty releases non-exclusive copyright interest for the Osler Institute to reproduce and distribute this material 
now and in the future, in printed or electronic media, with authorship acknowledged as “Edited by: [editor] from previous edition by 
[faculty]”.   
4.​ In order to provide our participants a polished, professional presentation and syllabus Osler requires that all presentation 
materials:  

  • Be provided on or before the due date - No exceptions. Said presentations will be the exact ones projected at course and printed.  
  • Have been recently reviewed and updated as necessary or at least once per year. 
  • Include a section with questions, answers, explanations and references for every lecture hour or topic. 

II. Faculty Expenses:   
1. ​Osler should receive expense forms with receipts within two weeks of the course.   
2.​ The Osler Institute will pay for travel, food and lodging for the faculty.   

  • Travel: Osler will provide the most economical air travel consistent with the faculty’s schedule. We will not reimburse for tickets purchased by faculty. Mileage will be paid at .50 cents per mile. Other ground transportation and parking fees incurred directly as a result of your participation on the faculty will be paid when a receipt is provided.   
  • Food and Lodging: Osler will provide at the course location. Your faculty coordinator will make reservations for you. 

 III. Audio and Video Royalties:   
The Osler Institute records sessions both for faculty review and to sell or rent. Payment for being recorded is included in the 
faculty honorarium. The Osler Institute will periodically pay royalties of ten percent (10%) of the sale price of their portion of the recording 
to faculty.   
IV. ACCME Content Validation Statement:  
 All clinical recommendations that Faculty make involving clinical medicine must be based on evidence that is accepted  
within the profession of medicine. All scientific research referred to in CME activity will conform to generally accepted 
standards of experimental design, data collection and analysis.   
V. HIPAA Compliance:   
Faculty will remove all patient identifiers (name, birth date, record number, etc) from their presentation materials and will not use 
identifiable photographs of patients, unless HIPAA compliant written permission has been obtained.   

This agreement will remain in effect until cancelled in writing by both parties or replaced with an updated version of the agreement.  I agree to the above and certify that: I am not subject to backup withholding and I am a United States person.
